Josephine Ortiz of the SA Food Bank Benefits Assistance Team will be on-site to help you through the process of screening, applying or renewing your benefits for SNAP, CHIP, WIC, Medicaid and more!

Documents to bring:
– Social security cards, ID or birth certificates for all household members
– Proof of Income: last 4 paystubs, Social Security Award Letter, etc.
– Rent receipt, lease agreement or mortgage payment
– Most recent bills for telephone, electricity, gas, water, insurance premium, medical bills and prescriptions
– Current bank statements, life insurance policies, real estate tax statements, etc.
– Immunization records: TANF and children’s Medicaid only
